Artemesia, Dean of the Page School, will have take and make bags that youth can pick up and do where ever they are on site. She will be set up with a table on site. She will also have a daily quest for the Youth with beads as prizes each day. See her for some quick Youth Activities!
Please remember if you're bringing a child to an event that is not yours, you must have the following. No exceptions. You will be turned away from gate and sent home if you do not have the following for the youth attending with you.
For the following, the policy you follow is for the state in which the event is happening. So if you are from Indiana attending an event in michigan, you must follow michigan's policy.
1. Youth Waiver Form signed by parent/legal guardian
Or for several children from the same family:
2. Medical Authorization for Minors signed by parent/legal guardian. (3 copies. One for parents, the adult attending the event, and the child.) This form must be presented at gate but stays on the adult. Gate does NOT retain these forms.
This must follow state policy as to how the form is completed.
Indiana and Kentucky state laws require notarization for a document of this type to be valid.
Michigan state law requires either a witness OR notarization for a document of this type to be valid.
Ohio and Illinois state laws do not require notarization for a document of this type to be valid.
